ANNAPOLIS - Maryland insurance commissioner appointee Ralph S. Tyler, for the second time in a week, announced a significant fine against an insurance company. In this case, the commissioner has imposed a $750,000 fine - the largest P/C fine in state history - on a national property-casualty insurer.
